Foreword
 

This Cardiovascular Disease Resource has been developed so that communities can benefit from the development of Early Intervention Screening programs that help identify risks of Heart Disease through their Health Services.

The Results and Tools contained within this Resource are based on "Heart of the Grampians" a Cardiovascular Disease (CVD) Program piloted in the Grampians Region in 1999.

The Resource incorporates a comprehensive Report, on the "Heart of the Grampians" Project and results of workplace health programs, in order to provide supportive evidence and recommendations for future, alike, programs. Tools that were developed throughout the "Heart of the Grampians" project and further updated for use in the "On Track" Program through the Mallee Track Health & Community Service (MTH&CS) in Ouyen are included. The Project in the Mallee Region is using new screening technology, sponsored by Pfizer Men's Health Tune Up Program, further advanced techniques and Best Practice Resources in early intervention to reduce CVD risks in remote rural areas.

"Given that approximately 8 million Australians spend a third of their day at work, it is important to ensure that Australian Workplaces have a positive rather than a negative impact on employee health".1

The "Heart of the Grampians" Program was conducted in workplaces as they are an ideal community venue, to develop early intervention programs that tackle and track the risk factors of heart disease for men and women.

We hope that our work makes it easier for you to achieve a better level of Preventative Health Care that raises the awareness of participants and helps them recognise their potential risk to Australia's biggest killer of men and women - cardiovascular disease.

We as an organisation, through our objective, commit to service innovation, change and flexibility. When challenging attitudes one must produce information that combines facts and clear benefits for considering change. Life itself is often disregarded, until a crisis confronts an individual, a family or community.

This Cardiovascular Disease Resource is a credible document, which enables communities and organizations to embrace change and progress to a more secure healthy life style.
